Important Dosing Notes

FDA-recommended limits:

Critical Rules for Children’s Dosing

  1. Know your child’s current weight — Weigh your child if unsure
  2. Use the correct product concentration — Concentrations vary between brands
  3. Use the provided measuring device — Kitchen spoons are inaccurate
  4. Never give more than 5 doses in 24 hours
  5. Wait at least 4 hours between doses

Infants Under 3 Months

Do not give acetaminophen to infants under 3 months without healthcare provider guidance. Any fever in this age group requires medical evaluation.

Calculating Total Daily Acetaminophen

The greatest risk of overdose comes from taking multiple products containing acetaminophen without realizing it.

Step 1: Identify ALL Acetaminophen Sources

Check labels for:

  • Acetaminophen
  • APAP
  • Paracetamol

Common combination products containing acetaminophen:

  • NyQuil, DayQuil (325-650 mg per dose)
  • Theraflu (650-1,000 mg per dose)
  • Excedrin (250 mg per tablet)
  • Tylenol PM (500 mg per dose)
  • Percocet, Vicodin (300-750 mg per tablet)

Step 2: Add Up Your Total

Example calculation:

  • Morning: 2 extra-strength Tylenol = 1,000 mg
  • Afternoon: Cold medicine dose = 650 mg
  • Evening: 2 extra-strength Tylenol = 1,000 mg
  • Bedtime: Sleep aid with acetaminophen = 500 mg
  • TOTAL: 3,150 mg (at the recommended daily limit)

Step 3: Stay Within Limits

  • Track every dose from every product
  • Set reminders for when next dose is allowed
  • Consider using a single-ingredient product to simplify tracking

When Fever Doesn’t Respond to Maximum Doses

If fever persists despite appropriate acetaminophen dosing:

Do NOT:

  • Exceed the maximum daily dose
  • Take doses more frequently than directed
  • Add additional acetaminophen-containing products

Do:

  • Ensure adequate hydration
  • Use physical comfort measures (light clothing, cool compress)
  • Consider alternating with ibuprofen (if appropriate and with guidance)
  • Contact a healthcare provider if fever persists more than 3 days or is very high

Special Situations

Fever with Vomiting

If the person vomits within 15-30 minutes of taking acetaminophen:

  • Wait 30-60 minutes for nausea to settle
  • May repeat the dose if vomiting occurred immediately

Fever with Dehydration

  • Acetaminophen is preferred over ibuprofen for dehydrated individuals
